The Moor Park & Sandy Lodge Residents Association annual subscription of £5.00 for the year 2008-2009 is due on 1st October 2008. A large number of members pay their annual subscription by Standing Order. This is a convenient payment method for both the member and the Association. If you would like to pay your annual subscription by Standing Order click here. Please print the form, complete as required and send it to the Treasurer at the address below so that the Association’s account number can be added. To help us with the administration and in the compilation of a new list of members, we would be grateful if you could complete the reference section on the Standing Order form. The reference is the first four letters of your surname followed by your house number and then the initials of your road, please click here for road codes, for example: - Name and address: Mr G Dossett, 8 Anson Walk Reference: DOSS8AW For those members wishing to pay by cheque please print off this page add your name and address in the space below and send it to the Treasurer with a cheque payable to Moor Park & Sandy Lodge Residents Association. Could you please ensure you write your name and address on the back of your cheque.
